The scope specifies a HEPA air filtration device for the Category 2 confined-space work, alongside elevated respiratory protection for the same area. No AFD appears in any equipment list on this job, and no photo shows one on site. If HEPA filtration is billed on the final invoice, there is no equipment record and no photo to support it. Route to: Project Manager — confirm whether the device was placed and never logged, or never placed at all. Those are different problems with different fixes.

2. The confined space never received the air mover the scope called for. One LGR dehumidifier is logged and photo-confirmed. The scoped air mover is absent from the log and from the photo record. Route to: Project Manager.

3. Deployment ran to roughly twice the scoped equipment count. Twelve air movers against six scoped, four dehumidifiers against two. This is a common and often correct field decision, since the initial scope is typically written before the affected area is fully assessed and actual placement should follow the drying chamber calculation, not the first estimate. It is not a field problem. It is a billing exposure: if the estimate still reflects six air movers and two dehumidifiers, a reviewer will cut the invoice back to the scoped count and the extra equipment days go unpaid. Route to: Estimator — the estimate needs to reflect actual placement, with the drying chamber math attached to justify it.

On photo timing. Every piece of equipment in the log has a matching photo. Placement in the hallway, living room, office and confined space is photo-confirmed within the same hour it was logged. The kitchen equipment is the exception: it was placed late on Jun 20 and first photographed on the morning of Jun 22, a gap of about 35 hours. The placement is supported, but not contemporaneously. A reviewer comparing log timestamps against photo timestamps will see that gap. Route to: Project Manager — worth understanding why, since the rest of the job was documented on the spot.

Confirmed clear: every logged item has photo support, unaffected areas are correctly empty in both the log and the photo record, and equipment removal dates are logged for every room.
